Otto earned a bachelor's degree in pharmacy from Virginia Commonwealth University and a Doctor of Pharmacy degree from Shenandoah University. He lives with his wife, Judith, in Stony Creek, Virginia. They have two daughters. Otto is a member of the Joyner Gray Yale Ruritan Club.

He lives with his wife, Judith Carol Hodge Wachsmann, and their two daughters in Stony Creek, Virginia. He is a member of the Joyner Gray Yale Ruritan Club and the Stony Creek Volunteer Fire Department.

Otto is a pharmacist and business owner. He owned Stony Creek Pharmacy in Stony Creek, VA, for nearly 20 years. He graduated from the Medical College of Virginia-VCU School of Pharmacy in 1986 and immediately started working as a staff pharmacist for Peoples Drug in Richmond. In 1989, he took a position at the A.H. Robins Company in Richmond as their first manager of managed health care and later served as their manager of new product planning. After a brief return to community practice, he joined the faculty at the Bernard J. Dunn School of Pharmacy at Shenandoah University and was named director of experiential education. In 2003, he returned home and purchased his father’s pharmacy, continuing the family tradition until 2021.
